Microsoft Forms
Microsoft Forms enables you to quickly and easily create a variety of forms to collect data and gather results.
If you are creating a Form to be used on a continuing basis, then build it inside a Microsoft Team. Using Teams, members of the Team can edit the form questions, and see all results. If you create a Form under your own Microsoft 365 account then it will be deleted when you leave, therefore this is not a suitable approach for ongoing work.

A simple, lightweight app that lets you easily create browser-based forms. You can use it to:
- Create accessible surveys, quizzes and polls
- Collect feedback and measure satisfaction
- Plan activities
Send to anyone
You can set who will be able to fill out each form (e.g., UofG people only, or anyone anywhere) and whether or not their responses will be anonymous.
Your invited respondents can complete your form through any web browser, even on mobile devices.
See real time results
As results are submitted, you can use built-in analytics to evaluate responses.
You can also export Microsoft Form data - such as quiz results - to Excel for additional analysis or grading.
Collecting personal data? You have extra responsibilities
If you will be collecting personal data through your form, you must comply with the Data Protection Act 2018 (GDPR) by designing privacy in from the start.
Privacy Notice
You must link to a relevant privacy notice at the start of your form, so your respondents can read it BEFORE they start filling in the form. If you haven't prepared a privacy notice yet, you can base it on the UofG template.
Data Protection Impact Assessment
If the data you intend to collect may be high risk, or your form has free text fields (where someone could enter more info than you asked for!), you must also carry out this assessment.
Don't get flagged for phishing!
Don't ask for sensitive personal information such as passwords, ID numbers, passport details or credit card numbers.

Supported service
Microsoft Forms is a supported service provided under the University's Office365 contract.
Do not use consumer services (eg Survey Monkey) for University work.
